New patients to the Clinic, are asked to complete a registration packet which will include:

Please Download & Print the completed forms and bring them with you to your appointment. You may also select the Submit by Email option to send the forms to our office so we have them ready for you ahead of time. If you do not have these forms completed, please arrive 15 minutes early for your appointment to complete the registration process.

We appreciate receiving 24 hours' notice if you are unable to keep your appointment. This will allow us to give another patient the opportunity to be seen.

When making appointments, our scheduling staff will need the following information:

  • The full legal name of the patient and, if applicable, the responsible party
  • The name of the physician or provider you wish to see
  • The nature of your health problem or why you wish to see the physician or provider
  • Your primary and secondary insurance coverage
  • Your phone number for confirming your appointment

What You Should Bring to an Appointment


Please bring:

  • Your insurance card and/or coupon
  • Your copay (if you have one) Note: Copays are required to be paid at the time of service
  • Change of address or phone number(s) if you’ve recently moved or made a change
